"Mixed experience in ED"

I’d like to express my gratitude for Dr. Ian at Fiona Stanley Hospital’s pediatric ward. He was attentive, compassionate, and quickly diagnosed my child chest infection, ordering an x-ray that revealed the issue right away. However, I must share my disappointment with the care we initially received.

Initially, we were admitted with my child, who had a high fever reaching 40 degrees. Despite the severity, the junior and senior doctors on duty didn’t conduct any blood tests or x-rays, and my child was sent home without a clear diagnosis. Two days later in the early morning , we returned to the emergency department, where triage nurse Rani was attentive, and Dr. Ian promptly assessed and treated my child.

Unfortunately, on the evening of that first presentation we had another disappointing experience with the triage nurse, who only took my child's temperature (was 40) and left us waiting without offering any paracetamol, despite their fever. As a nurse myself, I understand the challenges of staffing and the demands of a busy shift. However, it’s essential for pediatric cases, especially with high fevers, to receive prompt attention and empathy. Children are vulnerable, and even small actions, like administering paracetamol or notifying a doctor, make a huge difference.

Once again, I want to express our heartfelt thanks to Dr. Ian and Nurse Rani. Their kindness and empathy made a real difference for us. Dr. Ian in particular, was incredibly attentive, checking on my child multiple times and making sure we felt comfortable. He even asked if we had any further questions or requests and assured us that we could return if we had any concerns.

While I’m grateful for their care, this experience left me concerned about the healthcare system’s future as Australia’s population grows. If we cannot ensure sufficient attention in the medical field, it raises serious questions about how we will manage future needs.

I am sorry your child was unwell; I know this can be a distressing time for parents.

The decision to perform investigations such as blood tests and x-rays for patients presenting to the ED is influenced by a number of indicators such as vital signs, patient risk factors, localising clinical signs and not just the patient’s temperature. I apologise that this wasn’t explained to you during your child’s initial presentation to the ED as this would have been our expectation.

In addition, paracetamol is generally given to alleviate discomfort often associated with the fever rather than for the fever itself. During the paediatric triage process the nurse is making a quick assessment based on presenting history from the parent and limited observations including respiratory rate, breathing difficulties, heart rate and temperature. There is also a waiting room nurse allocated to the paediatric waiting room 24/7 to monitor patients waiting to access the main part of the ED. They are able to complete a full set of observations, offer first aid, fluid replacement and escalate concerns or medication needs to a doctor. It is difficult to comment on whether this occurred in your child’s case without their details.
