"Incredible Heartlink Nurses Care factor and support"

Just providing feedback on the great work that the amazing team of the heart link Nurses do in this case helping me to effectively help my mother with her heart condition with the CMC remote monitoring service.

It a big relief to know that the Community Management Centre...when my mother’s reading that I enter into the RPM HF APP is managed by nurses remotely instead of in person like in the hospital which can be difficult to keep doing it that way

Now that I am able to do the readings myself to help my mother and the reading go directly to the heart link team to reveal the condition and strength of my mothers heart condition it does ease so much pressure and concerns that I previously had.

I get regular calls to ensure my mother is doing ok and also advise on reading that at times may be a little concerning. The heart link team are amazing with the care factor that they provide to my mother

Thank you so much to the amazing Melissa, Meg, Susan the registered nurses Heart Link who have been incredible with their ongoing help with my mother and also helping me to understand the monitoring process as doing it at home is a great relief and less stressful on my mother
