"Waiting list for surgery"

About: Fiona Stanley Hospital / Advanced Heart Failure Department Fiona Stanley Hospital / Cardiology, CCU and Ward 4D

(as the patient),

I feel the mental side of being on a waiting list is far worse than the physical side, I worry about it everyday, I am scared, anxious and stressed. Here is my story so far:

I am on the wait list for cardiac surgery at Fiona Stanley Hospital, I have a leaky aorta heart valve deemed serious, which has caused an enlarged heart which is then causing the pulmonary heart valve to also leak, will also need 2 or 3 bypasses. Pre-op tests revealed I also have issues with my Carotid arteries in the neck which supply blood to the brain with both left and right >80% stenosis, blocked. I have read, that the estimated risk of stroke or total carotid occlusion is 46% per year in patients with asymptomatic carotid stenosis greater than or equal to 80%.

My Current Official wait time for Category 1 Urgent Cardiac Surgery is now almost 200 days, more than 300 days since referral. Although I have had my urgent category changed four times so far, was Category 1 mid last year, then changed to Category 2 a few days after that, changed back to Category 1 a couple of months later and have now been advised, although no letter to confirm that I am Category 2.  I was referred to the Vascular surgeon late last year, but not added to the vascular surgery wait list until late last month although no letter to confirm this has been received. I have not received a date for either surgery yet.

My Cardiologist at another hospital has been great and is also concerned about delay in surgery, writing in a letter to Fiona Stanley Hospital:

Their cardiac risk may escalate exponentially as their left ventricular function declines; and, I request our vascular and cardiothoracic surgical colleagues do what they can to expedite their surgery, to intervene before irreversible decline in ventricular function.

I am also happy with the medical teams I have seen at Fiona Stanley Hospital, they have all been helpful and professional. The only issue I have is the doctors give you an estimated time you are likely to wait for surgery, which appears to be no where near reality.

I understand that there are many patients on waiting lists at Fiona Stanley Hospital, and some may be in a worse condition than me, (although it is hard to imagine how) I know I need to wait my turn, but when I look at  the median waiting time for Cardiothoracic Surgery is 73 days and 90 percentile is 235 days, surely I must be getting close for urgent surgery which has a benchmark of 30 days.

Who is making waiting lists decisions at Fiona Stanley hospital? Doctors or Administrators/accountants/government officials?

Why has my surgery category been changed four times and does it mean every time it has changed I go to the bottom of the list for that category?

Dear Waiting666,

Thank you for taking the time to share your story with us. I am so very sorry to hear of your ill health and the understandable impact and stress the wait for surgery is causing you.

At Fiona Stanley Hospital we strive to deliver the best care in a timely manner, and I would like to sincerely apologise that this has not been your experience of our service.

We are currently experiencing significant emergency demand for Cardiothoracic surgery which is adversely affecting waiting times on our elective waitlist.

In relation to your personal case, I understand that you were originally listed as Category 2, and subsequently changed to Category 1 after a Surgical Consultant review.

I have been assured that your category on the Cardiothoracic waitlist has only changed once and your place on the waitlist has not been negatively affected by your change in category.

I can also confirm that you are currently on the Vascular Surgery waitlist as a CAT 1 and still within boundary.

The waitlist team are working with the surgeon to secure you a theatre date as soon as possible and you will be contacted by the team once confirmed.

I hope to reassure you that we do have robust processes in place to ensure the clinical safety for all our patients, including regularly reviewing waitlisted patients for clinical urgency and priority. Following these assessments our surgeons are then responsible for making booking decisions.

Please know how important your treatment is to us. I would encourage you, should any of your symptoms change or new clinical concerns arise, to contact the Cardiothoracic department on (08) 6152 3636 to discuss.

Thank you for sharing your story with us, I wish you all the best with your health and upcoming surgery.

Kind regards

Neil Doverty
