To Dianne, Belinda and Annie,

I am most grateful that you were able to include me in your TACP program, I am amazed at how many services are available on the program and fully funded. As the program ends I would like to thank each of you for your care, kindness, respect and friendly nature. You were always ready to help me with any problem I had and prepared to assist outside the box. I felt nothing was too much trouble for you.

Thank you for all the many ways you assisted me, for teaching me new skills, advocacy on my behalf and alerting me to goods and services. I have been most privileged to have you in my home, and for all the many tasks you assisted me with. Home hospital has been a very healthy and beneficial program.

I am most grateful for your kindness and concern for me.

Thank - you for taking the time to write to us and share your amazing experience with the Transitional Aged Care Team. We hope that your elderly parent is continuing to recover and rehabilitate following their critical illness.

We gratefully acknowledge your appreciation of the comprehensive individually tailored rehabilitation program provided by our clinical staff. It's so pleasing to read that our team were able to support you in your recovery. They are a highly trained and empathetic team that have helped many families like your own and made an enormous difference to so many other families in our community.

We will ensure that Dianne, Belinda and Annie, are informed of your great appreciation and thanks for the care that they delivered to you. It is so pleasing to read that our TACP were able to help and support you during this very difficult health journey.

We hope that you continue to have ongoing benefits from the rehabilitation program. Thanking you for sharing this positive health story with us it is greatly valued and appreciated.
