"No discharge summary created"

About: Fiona Stanley Hospital / General Medicine Department, & Ward 6C

(as the patient),

I was covid positive and was admitted to ward 5D at FSH, when I was no longer positive I was moved to ward 6C. I believe no discharge summary was created for this admission at all. It is not available on My Health Record, my GP doesn't have a copy, Freedom of Information says the summary doesn't exist in my medical records. Where is it?

It has been almost 4 weeks from discharge from that admission. I was admitted again that month to 5B then 6C and it seems there is no record that I was there with covid two weeks prior as there was no discharge summary on their system! I've now been discharged again and no discharge summary uploaded or available to my GP either. Again. My medications have changed because of adverse reactions to the ones I was prescribed and my GP will not have that communicated as to what happened and what to do next. In my opinion, due to lack of communication from the hospital on two occasions now. 

I need those discharge summaries, why were they apparently not created? I've rang FSH asking, they put me through the FOI, I recall they said the summaries don't exist, they weren't created.
