"The 'Fabulous Four' health professionals"

My family and in particular my darling relative would like to personally thank and acknowledge the "Fab Four" health care professionals, Tara (Social Worker), Kelly (Physiotherapist), Ashley (Occupational Therapist) and Kim (Community Nurse) from the Transitional Aged Care Community Team from the Prince of Wales Hospital who were inspirational and crucial to my relative's home care rehabilitation and subsequent recovery.

The above four wonderful health professionals recently visited and cared for my relative in their home where they collaboratively designed a personalised recovery rehabilitation program to assist and restore their return to full health and normal activity.

It is with great gratitude and heartfelt appreciation, that we wish to acknowledge: Tara, Kelly, Ashley and Kim, who not only supported and guided my relative to better health but also gave incredible empathetic care and support to myself and my sibling during a very difficult and distressing time.  

Their expertise, knowledge, insight, compassion and encouragement will never be forgotten. They made a life-changing difference to our family as I believe they have done to so many other families that they engage with.

In my opinion, this team has the model for success for ensuring wellness in our community and has a unique formula to facilitate sustainable health and well being.
