"Confusing information on general information form"

About: Fremantle Hospital and Health Service / Outpatients

(as the patient),

On patient info paperwork received prior to me attending my appointment it stated that if I took Clopidogrel or Warfarin (?) I should consult my Doctor!!!  My Doctor had referred me to the Urology Dept and was quite aware of medications that I take so I tried to phone the Urology department to enquire if I needed to cease taking my tablets, being blood thinners/anticoagulants, prior to my procedure.  I could not get any intelligent response from anyone that I spoke with at Fremantle Hospital, they were all very polite etc but continued to put me on a merry-go-round of hospital departments  until I got back to the main switch.

I feel that the wording used on the form that gave instructions etc should be changed to read in a more simple, easy to understand way ;   either I continue to take my medication or I stop prior to my procedure!!
