"My Rehabilitation experience with the Transitional Age Care Program Team"

About: Prince of Wales Hospital / Transitional Aged Care Program

(as the patient),

I would like to thank the TACP team from Prince of Wales Hospital who worked hard and consistently care and support me for me to gain the  strength and confidence soI  can be independent once more and live on my own in the safest way possible.

I believe I was so lucky to be given such a wonderful TACP team:
Physiotherapist and Case Manager - Jess
Occupational Therapist - Holly
Registered Nurse - Faye
Social Worker - Stephanie
Community Aid - Hanna.

I believe the team has shown professionalism and is very respectful. Each and everyone is very knowledgeable in their own field. This is shown by their individual skills and experiences. The best of all is they are very caring and value my opinion. Them understanding me at a personal; level makes it easy for me to open up and achieve my goals quicker in the most efficient way.
Based on my experience, I sincerely would recommend them to other patients and will treasure their care and help in my heart.

Thank you for taking the time to acknowledge your positive experience with the POWH TACP team.

It is wonderful you received such professional care by the team making special mention to Jess, Holly, Faye, Stephanie and Hanna.

But what is exceptionally great to see is the person centered care you received which has supported you to reach your goals quicker.
